Our Aging & Disability Care Management team provides in-home intensive care management for older adults and people living with disabilities in Ramsey and Hennepin counties.
Professional care managers assess clients’ needs, develop individual care plans, coordinate services, and advocate on their behalf.
In 2023, 96% of older adult clients were able to stay in the home of their choice and delay entrance to assisted living facilities.
